  • Log in to the National Voters Services Portal, the Official Website for services provided to voters.
  • Click on the "Correction of entries in electoral roll" section after navigating to it.
  • A brand-new page loads. Choose "Form 8" from the page's list.

  • Fill out the form with the following information. Fill out the form with the following information. State and the legislative district in which you are registered.   • Select the information that needs to be modified or rectified next. In this specific instance, you will need to click on the "My Name" button to make sure that the procedure only modifies the name on your voter ID.
  • Put in the location where you're sending the request (city).
  • Mention the day that you are requesting a change to the name on your voter ID.
  • Fill in your contact details (mobile number, email ID).
  • Click the "Submit" tab after you have checked the data you have provided.

Your name as it appears on your voter ID card may be changed or corrected by submitting an application form together with required documentation.
